This is an iPhone 5 crochet case/cozy/cover/sleeve. You can also use this pattern for iPhone 4, the difference in size is not significant and there are instructions in the pattern how to change the size for iPhone 4, if you need that.
Finished size
Approximately 5.1” (13 cm) long without legs, 2.8” (7 cm) width.
Gauge 5.5 sts = 1”.
